Overview:

The Homelessness, Addiction, and Recovery Treatment (HART) Hub is a collaborative initiative designed to provide a seamless continuum of care through a culturally responsive and healing-centered model.  Tailored to meet the needs of individuals facing homelessness, and mental health challenges, the Kenora HART Hub serves as a vital anchor for support within the Kenora geographical area, and surrounding communities.  By delivering coordinated, compassionate, and evidence-based services, the HART Hub ensures equitable access to care.  This approach integrates holistic and culturally grounded practices, to promote stability, healing, and long-term recovery for members who are most vulnerable within our communities.

 

Reporting to the HART Hub Operations Coordinator the Transportation Coordinator will be responsible for coordinating transportation services to clients of the HART Hub who require transportation assistance.  The Transportation Coordinator will coordinate transportation services for clients who are receiving services from the co-located integrated care team that consists of both the internal HART Hub services operated by CMHA Kenora Branch as well as external partners.  The

co-located onsite integrated care team services will consist of low barrier access to a range of services through case management and transition support, nursing, peer support, transportation support, crisis stabilization support, addiction services, land-based cultural care, connection and referral pathways to supportive housing support, and onsite administrative support.



Qualifications:

B.A. in behavioral science preferred

Experience in working in the transportation field or related

Experience working with individuals who are homeless and marginalized who also have mental health and addiction struggles and/or demonstrated ability to work with this population

Personal qualities of empathy, understanding, and patience

Good interpersonal communication and relationship building skills

Ability to communicate professionally with community agencies

Knowledge of transportation resources within Ontario

Cultural competence for diverse populations

Understanding and applied knowledge of Recovery Model

Understanding of trauma informed practice and implementation

Good documentation skills

Ability to learn and understand online scheduling

Proficient computer skills

Demonstrated understanding of personal space and ability to role model healthy boundaries

Ability to manage conflict and crisis situations

Demonstrated time management skills

Ability to work independently as well as in a team environment

Creative problem solving

Results oriented

First aid & CPR/AED, Mental health first aid, and NVCI or willing to obtain

Valid Driver’s License

 

Duties:

  • Adherence to all policies and procedures of Canadian Mental Health Association, Kenora Branch
  • Collect transportation service supports available for clients who are requesting support for transportation coordination of the HART Hub (Non-Insured Health Benefits)
  • Provide information and answer questions regarding options, for appropriate travel arrangements for individuals accessing the HART Hub
  • Provide assistance to internal and external programs in arranging transportation for out of town appointments such as (doctor’s appointments, dental appointments, vision appointments, counselling appointments, legal appointments, court appearances, return to home)
  • Ensure transportation initiatives on behalf of clients of the HART Hub are maintained by providing instructions to drivers, reviewing special needs of clients where applicable for support needs
  • Ensure proper documentation is provided to transportation services that clients are receiving or participating in and follow the policies and procedures of the Canadian Mental Health Association, Kenora Branch
  • Complete and submit financial expenditure reports that are applicable to any approvals for transportation assistance approved by the HART Hub Operations Coordinator in consultation with the Finance & Administration Manager and the Chief Executive Officer
  • Prioritize requests for transportation coordination requests received
  • Maintain a register for all transportation requests per fiscal year to understand the needs of those accessing the HART Hub
  • Ensure clients who are accessing the HART Hub are triaged to the Clinical Service Hub Team where appropriate (Clinical Service Hub Team Lead, Service Navigator and Peer Support)
  • Ensure clients are triaged to appropriate services that are co-located onsite by the integrated care team services which will consist of low barrier access to a range of services through case management and transition support, nursing, peer support, transportation support, crisis stabilization support, addiction services, land-based cultural care, connection and referral pathways to supportive housing support
  • Develop an integrated Recovery Plan in partnership with the identified integrated care team and strategies to support the achievement of goals and objectives of clients of the HART Hub as it relates to transportation needs of clients
  • Assertive engagement with clients of the HART Hub to connect them to transportation services as identified
  • Make appropriate recommendations for transportation services and support services for clients requiring transportation supports
  • Consult and collaborate with the integrated care team regarding the support and integrated recovery planning for HART Hub clients
  • Liaise with the integrated care team to assess client transportation needs within the community
  • Encourage understanding of the transportation and support needs for clients by developing and maintaining working linkages with law enforcement, primary care, human service agencies, and with informal community resources
  • Continuous assessment of risk
  • Continuous assessment of risk related to the clients transportation needs
  • Advocacy or linking of clients with appropriate transportation services, supports and resources to meet the goals and objectives of the integrated recovery plan
  • Conducting regular formal or informal evaluation of client satisfaction, achievement of goals, reassessment of service requirements and or needs in consultation with the client
  • Direct provision of side-by-side support with clients where appropriate
  • Provision of supportive counselling where appropriate
  • Assist clients and provide modeling for problem solving and priority setting
  • Referral to community agencies and services where appropriate
  • Maintenance of client files consistent with policies and procedures of Canadian Mental Health Association, Kenora Branch
  • Outcome monitoring and evaluation as directed by the Chief Executive Officer
  • Professionally representing Canadian Mental Health Association, Kenora Branch
  • Drafting all necessary correspondence and reports regarding clients and Team Lead reports
  • Participation in administrative and office functions, including ongoing maintenance of office space
  • Participation in educational and professional development opportunities
  • General administration duties
  • Communicate changes and new rules/regulations to staff; ensure all operation and program policies and procedures are being followed
  • Attend staff supervision, performance review and team meetings as directed by the HART Hub Operations Coordinator and Chief Executive Officer
  • Attend education/training opportunities as required
  • Continue to build relationships within the community through one-on-one networking, group meetings, and public outreach as directed by the HART Hub Operations Coordinator and Chief Executive Officer
  • Participate on specific committees or working groups as assigned by Chief Executive Officer
  • Provide a safe environment by ensuring adherences to Workplace Health and Safety
  • Actively participate in the development of sustainability of the ongoing Accreditation Standards, Required Organizational Practices and Quality Improvement projects
  • Regulations and Policies, Infection Control Guidelines, WHIMIS and Safe Work Procedures, including but not limited to Fire, Disaster and Evacuation Plans
  • Ensure client safety and client safety activities are emphasized throughout orientation of new staff members
  • Ensure client safety and clients safety activities are emphasized to the co-located integrated care team
  • Support an environment which avoids, prevents and corrects all activities or actions which may result in an adverse outcome in the delivery of mental health services
