As a vital member of our caregiving community, you will assist with daily activities, monitor health status, and facilitate transportation to appointments and community outings. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Observe and document patient conditions accurately using electronic health record (EHR) systems while maintaining strict confidentiality in accordance with HIPAA regulations Support medication management by administering prescribed medications following medical terminology guidelines and safety protocols Conduct vital signs assessments such as blood pressure, pulse, temperature, and respiration to monitor health status Provide specialized care for clients with dementia or Alzheimer’s disease, including memory care techniques and end-of-life care when necessary Safely transfer clients using patient transfer methods like Hoyer lifts or other assistive devices while adhering to proper body mechanics and heavy lifting safety standards Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and meal preparation; ensure proper sanitation during caregiving activities Qualifications Certified Nursing Assistant (CNA) or Certified Medical Technician (CMT) certification preferred; relevant caregiving experience highly valued Proven experience working in assisted living facilities, nursing homes, or home healthcare environments with geriatric or disabled populations Knowledge of medical terminology, catheter care, wound management, and hospice or palliative care practices Familiarity with EMR/EHR systems for accurate medical documentation and client records management Strong understanding of home health care regulations, patient transfer methods, and patient personal care assistance techniques Ability to work independently with excellent client communication skills; compassionate approach to patient care is essential Valid driver’s license with reliable transportation; ability to assist clients with transportation needs confidently CPR and First Aid certification required; Basic Life Support (BLS) training preferred Physical stamina for heavy lifting tasks and long periods of standing or walking; experience working with individuals with disabilities is a plus Join us in delivering exceptional home health and community care services!
