Trinity Care LLCJob Summary Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) at the Crisis Receiving Center, where you will play a vital role in providing compassionate, high-quality care to individuals experiencing mental health crises and complex medical needs. Duties Patient Monitoring and Care: Administer medications as prescribed for detoxification or maintenance therapies and monitor clients for signs of withdrawal or complications. Health Assessments: Regularly assess and document vital signs, including blood pressure, heart rate and temperature, and provide physical assessments to identify health changes that may require further medical attention. Medication Management: Safely administer prescribed medications, observe for potential side effects, and collaborate with the healthcare team to adjust treatment plans as needed. Patient Support and Education: Educate clients on substance use recovery, health management, and strategies for maintaining sobriety. Provide emotional and physical support during the detoxification and recovery phases. Documentation and Reporting: Maintain accurate, timely, and confidential patient records. Document all aspects of client care, including medication administration and any changes in condition, while ensuring compliance with regulatory guidelines. Team Collaboration: Work closely with a multidisciplinary team including counselors, social workers and physicians to develop and implement individualized care plans tailored to each client. Crisis Intervention: Respond to emergency situations, such as withdrawal complications or medical crises, and provide first aid when necessary, ensuring client safety at all times. Health and Safety Compliance: Adhere to infection control policies, safety protocols and legal/ethical standards. Support a clean and safe environment for all clients. Experience Valid LPN license in the Commonwealth of Virginia Completion of an accredited practical nursing program Current CPR and First Aid Certification Experience or a strong interest in working with individuals recovering from substance use disorders is preferred Excellent communication and interpersonal skills, with the ability to work collaboratively in a team environment Knowledge of substance use disorders, detoxification processes and recovery-oriented care is a plus Ability to maintain professionalism and empathy in challenging or crisis situations Must pass a background check and drug screening.
