In this role, you'll provide comprehensive pharmaceutical care through routine and emergency drug screening, information, processing, and dispensing services for various drug distribution systems to the facilities. You'll also be responsible for ensuring that standards are maintained, policies carried out, and objectives accomplished. Consistently ensure that the appropriate drug product reaches the customer safely. Location : Pharmscript Home Delivery Pharmacy: 80 Clark Dr, Unit 2BC, East Berlin, CT Hours : Monday-Friday, 40 hours between the schedule of 12:30pm-11:00pm Primary Responsibilities: Review and verify prescription orders to ensure accurate dispensing, medication safety, and regulatory compliance for long-term care residents. Conduct medication regimen reviews and collaborate with prescribers, nurses, and other healthcare professionals to optimize resident medication therapy. Provide clinical support and drug information, including recommendations related to dosing, therapeutic alternatives, drug interactions, and adverse effects. Ensure compliance with all federal, state, and facility regulations, including controlled substance management, documentation, and recordkeeping requirements. Oversee medication distribution processes, inventory management, and timely delivery of medications to assigned facilities. Participate in quality assurance, medication safety, and performance improvement initiatives to promote positive resident outcomes. Support facility staff through education, consultation, and assistance with medication-related questions and concerns. Required Qualifications: Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited college or school of pharmacy required. Active and unrestricted pharmacist license in the state of practice (Connecticut) Excellent clinical judgment, communication, and interpersonal skills with the ability to collaborate effectively with interdisciplinary healthcare teams.
