Job type: Full-time, W-2 About Old Mission Advanced Care Old Mission Advanced Care delivers home-based wound care in geographic "pods" — small territories where one clinician sees a full day of patients with minimal windshield time. Density is destiny in home care, and the Dispatcher is the person who makes density real. We're launching our Texas and Florida pods now. The role You turn care plans into drivable days. Our Nurse Navigators set each patient's standing weekly appointment window; you build and publish the daily routes inside each pod, keep our 24–48-hour promise for acute referrals, activate urgent coverage when a pod loses its clinician, and manage the exceptions — illness, weather, a patient who isn't home — without the day collapsing. What you'll do Build and publish next-day routes for every active pod — stable templates built on standing windows, confirmed by every clinician before the day starts Own the exception queue in real time: re-sequence, swap, escalate Slot acute referrals inside 24–48 hours, even when it costs route elegance — the promise outranks the template Maintain pods inside the routing platform (member adds/moves, territory assignments) and report routing truth weekly: visits per day, drive time per visit, template stability Handle member exception calls (reschedules, delays) directly — in English and Spanish What you bring Demonstrated experience running a route-optimization platform (OptimoRoute, Route4Me, or similar) in a live operation — you will own pod maintenance in the platform from day one Bilingual Spanish/English required — exception calls are part of the job, and our members must understand the person calling 2+ years in dispatch, scheduling, logistics coordination, or home-health operations Calm, exact communication under time pressure; fluency with maps, drive-time tools, and spreadsheets Home-health or field-clinician scheduling experience is a plus Pay & benefits — stated plainly Base salary 58,000-72,000 — we recruit toward the top of the band for proven routing-platform experience Quarterly variable pay (target 12% of base) tied to route performance and enterprise results — funded from collected results and explained in full in your offer documents Health, dental, vision (100% of employee premium), 401(k) with 4% match after 90 days, PTO with planned dispatch coverage Who this is not for: passive schedulers who wait for the phone to ring.
