MAJOR FUNCTION:
The Patient Flow Nurse Coordinator is responsible for coordinating high-acuity patient movement, supporting system capacity management, and removing barriers to patient flow across the health system.
The Patient Flow Nurse Coordinators role provides clinical oversight for complex transfer cases, including high-acuity and behavioral health patients, and serves as a key resource for real-time decision-making related to patient placement and throughput. The Patient Flow Nurse Coordinator also functions in an expeditor capacity, proactively identifying and resolving barriers to discharge and patient progression.
In addition, this role supports transfer center operations and bed coordination during periods of high volume, ensuring safe, efficient, and timely patient movement across the continuum of care.
